Heart failure is serious. But, the good news is you can often manage the symptoms to prevent your heart failure from getting worse, and to stay healthy and out of the hospital. Here’s how:

  • Make sure you know the common symptoms of heart failure.
  • Watch your symptoms – every day – and call your clinician if your symptoms change or get worse.
  • Work with your clinician to create a plan to help you control your symptoms. The four key parts of this plan are:
    1. Diet
    2. Exercise
    3. Lifestyle changes
    4. Medication

Follow this plan closely to stay as healthy as possible. Ask your clinician to explain things that aren’t clear to you. Always check with your clinician before you make any changes.
